Layout & styling requirements (implement in native Form settings):

CRM integration instructions (post-creation steps if connector exists):
1) Enable the Forms CRM connector in Integrations/Forms for this account.
2) Enter required API credentials in the account Integration settings.
3) Map fields: first_name -> First Name, email -> Email.
4) Save the mapping and run a test submission to confirm contact creation.
Notes: I disabled this custom-code block (hidden on desktop & mobile) because I could not create a native Form element in this single-step API update. Please create the native Form element in the builder using the schema above and then remove this placeholder element.
